Registered Nurse
We have an exciting new employment opportunity for a Home Health Registered Nurse. The Home Health RNs are responsible for ensuring high-quality care for all patients.
Responsibilities:
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates patient nursing needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
- Performing field visits.
- Reviewing documentation to ensure regulatory compliance.
- Assessing the needs of patients & families, providing care per physician order, and addressing symptoms.
- Collaborating and communicating with the Interdisciplinary Team including physician when appropriate.
Qualifications:
- Current RN license
- Very strong clinical skills.
- Prior Home Health / Home Care nursing experience preferred, but training can be provided.'
License/Certification:
- BLS
- RN
Licensed Vocational Nurse
We have an exciting new employment opportunity for a Home Health Licensed Vocational Nurse. The Home Health LVNs are responsible for ensuring high-quality care for all patients.
Responsibilities:
- Performing field visits.
- Reviewing documentation to ensure regulatory compliance.
- Assessing the needs of patients & families, providing care per physician order, addressing symptoms, and ensuring comfort.
- Collaborating and communicating with the Interdisciplinary Team including physician when appropriate.
Qualifications:
- Current LVN license in good standing.
- Very strong clinical skills.
- Prior Home Health / Home Care nursing experience preferred, but training can be provided.'
- License/Certification:
- LVN
- CPR Certification
- BLS Certification
Home Health Aid
We have an exciting new employment opportunity for a Home Heath Aid. The HHAs are responsible for ensuring high-quality care for all patients.
- General Duties (varies on a per case basis):
- Personal Care: Bathing, dressing, grooming, toileting, walking and transferring assistance, Dementia/specialty care and safety care/monitoring.
Requirements:
- Home Care Aide Registry, and Clearance • TB Clearance
- Valid driver’s license
- A reliable vehicle and current insurance'
- License/Certification:
- CNA
- Certified Home Health Aide (Preferred)
